ECL services may be delivered through telehealth. Telehealth has benefits and limitations and may not be appropriate for every condition or patient.
You are responsible for providing accurate information regarding your identity, physical location at the time of treatment, medical history, medications, symptoms, and other information reasonably requested by your healthcare provider.
Services may only be provided where the treating provider is appropriately authorized to practice and where the service is legally permitted.
5. No Guarantee of Treatment or Prescription
Submitting information, completing laboratory testing, paying for a consultation, or requesting a particular treatment does not guarantee that a medication, prescription, therapy, or treatment will be provided.
All clinical decisions are made by appropriately licensed healthcare professionals based upon their independent professional judgment. A provider may determine that a requested treatment is inappropriate or that additional evaluation is required.
6. Prescription Medications
Prescription medications are provided only when an authorized healthcare provider determines that they are medically appropriate.
ECL does not guarantee the availability of any medication, formulation, dose, pharmacy, or treatment. Medication availability may change due to regulatory requirements, pharmacy availability, supply conditions, or clinical considerations.
7. Laboratory Testing
Certain services may require laboratory testing before treatment and periodically during treatment. Patients are responsible for completing required testing and follow-up as directed. Failure to complete required monitoring may result in treatment being delayed, modified, or discontinued.
8. Risks and Results
Medical and wellness treatments involve potential risks and benefits. Individual outcomes vary. ECL does not guarantee specific medical, weight-loss, hormonal, performance, longevity, cosmetic, or other outcomes.
9. Fees and Payment
You agree to pay all fees disclosed to you before purchasing services. Certain services may involve separate charges for consultations, laboratory testing, medications, pharmacy fulfillment, shipping, memberships, or ongoing monitoring. Unless expressly stated otherwise, ECL services are not represented as being covered by insurance.
10. Cancellations, Refunds and Memberships
Specific cancellation, refund, subscription, and membership terms disclosed at the time of purchase will apply to the applicable service. Amounts already paid for completed consultations, services already rendered, laboratory testing, customized or dispensed medications, or other nonrecoverable expenses may be nonrefundable where permitted by law.
